I the undersigned parent or legal guardian, voluntarily permit my child, named below to participate in virtual reality activities at Dragon Lady Entertainment Inc., understanding that VR activities involve inherent risks including falls, collisions, loss of balance, motion sickness, dizziness, eye strain, and other injuries. I have had the opportunity to ask questions and understand these risks. I voluntarily assume them on behalf of my child. To the fullest extent permitted by Ontario law, I release and discharge Dragon Lady Entertainment Inc., its owners, officers, employees, and agents from claims arising from my child's participation, except where liability cannot be excluded under applicable law. My child will follow all safety instructions, and the arcade may stop participation if staff believe it's unsafe to continue. I confirm that I am the parent or legal guardian of the child and have the legal authority to sign this agreement.

The parent, legal guardian, or designated responsible adult acknowledges that all birthday parties at Dragon Lady Entertainment Inc. must be supervised by an adult 18 years of age or older for the duration of the event. The supervising adult is responsible for the conduct and safety of all participants in their party, including ensuring participants follow staff instructions and all posted safety rules. Dragon Lady Entertainment Inc. does not provide childcare, and, the supervising adult accepts responsibility for all participants in their group.
